WHAT IS TINEA CAPITIS
Tinea capitis is a fungal infection of the scalp. It is most commonly seen in children, particularly between the ages of 3 and 10. The condition is caused by dermatophyte fungi that infect the scalp skin and hair shafts.
Tinea capitis often starts off as red bumps which easily spread. It can progressively spread, leading to scaling, hair loss, and scalp discomfort and may form a ring shape.
In more severe forms, inflammation can become significant (lymph nodes may be enlarged) and affect overall wellbeing.
WHAT CAUSES TINEA CAPITIS?
Tinea capitis is caused by a group of fungi known as dermatophytes. The most common causative organism in North America is Trichophyton tonsurans.
The infection is contagious and spreads easily through direct contact or shared objects such as hair brushes, combs, hats, pillows, and towels. Close contact in households, schools, or childcare settings increases the risk of transmission.
HOW IS TINEA CAPITIS DIAGNOSED?
- Diagnosis is usually made through clinical examination of the scalp.
- Additional tests may include
- Scalp scrapings or hair samples sent for fungal testing
- Use of a special examination light in selected cases to help identify fungal involvement
Early diagnosis is important to prevent complications and permanent hair loss.
WHAT TREATMENTS ARE AVAILABLE FOR TINEA CAPITIS?
Tinea capitis must be treated with oral antifungal medication. Topical treatments alone are not effective.
- Treatment typically includes
- Oral antifungal therapy for 4 to 6 weeks, most commonly terbinafine or griseofulvin
- Careful administration of medication in children, sometimes crushed and mixed with food as advised
- Avoidance of acidic foods when taking terbinafine, as they interfere with absorption
If inflammation is severe, a short course of oral corticosteroids may be prescribed to reduce swelling and discomfort.
Household members may be advised to use antifungal shampoos such as ketoconazole to reduce fungal spread.
WHAT WILL HAPPEN TO MY HAIR IN THE FUTURE?
When tinea capitis is diagnosed early and treated appropriately, full hair regrowth is expected.
Delayed diagnosis or inadequate treatment can lead to scarring and permanent hair loss, particularly in severe inflammatory cases.
