WHAT IS DISCOID LUPUS ERYTHEMATOSUS (DLE)
Discoid lupus or “DLE” is a chronic inflammatory condition of the scalp that often results in permanent or scarring hair loss. Individuals with DLE often first notice itchy pimples on the scalp that gradually heal but leaving scarred areas where hair cannot regrow.
Most individuals who have scalp DLE do not have systemic lupus erythematosus (SLE), which is a more serious type of disease affecting many organs in the body.
While the majority of the patients with DLE are healthy, approximately 5% of DLE patients ultimately develop SLE.
HOW IS DLE DIAGNOSED?
Our expert trichologist Donia Maria Messeh will perform a throrough evalutation by clinical examination, a scalp analysis by trichoscopy. And often a scalp biopsy will be performed to confirm the diagnosis.
WHAT TREATMENT OPTIONS ARE AVAILABLE?
There are many treatment options for DLE. There is no single approach to treating DLE, treatment is always personalised based on the extent of the condition and the individual response. The aim is calm inflammation, prevent further scarring, and protect the reminaing hair.
Treatment options may include:
- Topical corticosteroids in creams, lotions, or medicated shampoos to reduce inflammation
- Corticosteroid injections into affected scalp areas for targeted control
- Oral medications such as hydroxychloroquine to manage immune activity
- Advanced systemic therapies like mycophenolate or oral retinoids for resistant cases
Your exact treatment plan will depend on the severity of scalp involvement, your medical history, and how your scalp responds over time. Regular follow-up is essential to monitor progress and adjust the approach as needed.
If treated early, it may be possible to shut off the DLE or significantly reduce its progression. However, the condition can reactivate at any time, so ongoing monitoring is important.
It’s also important to note that any scarring that has already occurred is usually permanent and cannot be reversed. In some cases, hair transplantation may be considered to restore hair in scarred areas—but only once your dermatologist or hair loss specialist is confident that the disease has completely stopped or “burned out.”
