WHAT IS ACNE KELOIDALIS NUCHAE?
Acne keloidalis nuchae (AKN) is a scarring skin condition that develops on the back of the neck. It is most common in young Black men but can affect anyone, including women. It appears as itchy bumps that may be painful and sometimes drain fluid.
WHAT CAUSES AKN?
The exact cause is unknown, but it is thought to involve inward growth of curved hairs. This may be triggered by very short haircuts or repeated irritation from tight collars or sports gear.
HOW IS IT DIAGNOSED?
Generally the diagnosis is made by clinical examination (examination of the patient)
If there is draining fluid, a swab will be taken for testing.
WHAT TREATMENTS ARE AVAILABLE FOR AKN?
Advised is to avoid very short haircuts; grow hair slightly longer and avoid tight-fitting collared shirts when possible.
Medical options include:
- 2% clindamycin powder in DILUSOL
- 2% hydrocortisone cream
- Short-term use of potent topical steroids (e.g., clobetasol)
- Tretinoin 0.01% or 0.025% with a topical steroid
- Topical chloramphenicol (500 mg in 30 g fluocinonide cream)
- Oral antibiotics (e.g., doxycycline) if drainage is significant
- Steroid injections (may lighten skin for 6–12 months)
- Surgical removal of bumps by deep punch excision or shave followed by steroid injection
- Isotretinoin (Accutane) for severe, treatment-resistant cases
