Central centrifugal cicatricial alopecia

WHAT IS CENTRAL CENTRIFUGAL CICATRICIAL ALOPECIA)?

Central centrifugal cicatricial alopecia (CCCA) is a form of scarring (permanent) hair loss, mostly affecting women of African descent or darker skinned patients. CCCA typically begins in the mid-to-late 30s. Affected individuals may have no symptoms, while others experience an itching or burning sensation in the central region on the scalp (crown). Hair loss usually starts in this central region and gradually spreads outward in a centrifugal pattern, leading to progressive and irreversible hair loss due to scarring of the hair follicles. There is loss of follicular openings, so the scalp often appears shiny.

WHAT CAUSES CCCA?

The exact cause of CCCA is still unknown. However, it is hypothesized that certain hair grooming practices, such as braids, weaves, cornrows, and the use of chemical relaxers, particularly when started at a young age, may contribute to its development in genetically susceptible individuals.

HOW IS CCCA DIAGNOSED?

Diagnosis is primarily clinical, based on a thorough scalp examination by our expert trichologist Donia Maria Messeh. A scalp biopsy may be performed to confirm the diagnosis, but it is not always necessary.

WHAT ARE MY TREATMENT OPTIONS?

Treatment for CCCA aims to halt progression and reduce inflammation. Options include topical or injected corticosteroids, oral agents such as doxycycline or hydroxychloroquine, and in some cases PRP therapy. Gentle hair care practices are essential, and hair transplantation may be considered if the disease has been stable for at least two years.

DNA testing may also be included, offering valuable insights into each individual’s unique genetic profile and allowing for the creation of truly bespoke, highly effective treatment strategies.

Every consultation begins with a thorough evaluation of the hair, scalp analysis by trichoscopy, lifestyle, medical history, current routines, and emotional wellbeing. We take the time to listen and we take time to identify underlying triggers, whether it being genetic, hormonal, nutritional, environmental, before designing a tailored treatment plan that may include advanced therapies, topical or systemic treatments, and supportive care.
