WHAT IS FRONTAL FIBROSING ALOPECIA?
Frontal fibrosing alopecia (FFA) is a scarring type of hair loss that most commonly affects postmenopausal women (around 86%), although it can also occur in younger women and, more rarely, in men. FFA is characterised by recession of the frontal hairline, thinning at the sides of the scalp, and, in some cases, loss of the eyebrows, eyelashes, and body hair.
Many patients notice itching along the frontal hairline, while others may experience burning, tenderness, or pain in the affected areas. On examination, there may also be redness and scaling of the scalp.
Because it is a scarring alopecia, the hair loss in the affected areas is usually permanent. However, studies have shown that with appropriate treatment, some regrowth may be possible in selected cases, particularly when therapy is started early and inflammation is controlled.
HOW IS FRONTAL FIBROSING ALOPECIA DIAGNOSED?

The diagnosis of frontal fibrosing alopecia is supported by several features, including areas of scarring hair loss, redness and scaling around the hair follicles.
A scalp biopsy may be performed to confirm the diagnosis, biopsy findings will be showing inflammation, loss of the sebaceous glands, and scarring around the hair follicles.
WHAT TREATMENT OPTIONS ARE AVAILABLE FOR FRONTAL FIBROSING ALOPECIA
The main goal of the treatment is to stop the disease from progressing. Several treatments may help slow the progression of FFA and reduce symptoms, these include:
- Oral medications such as finasteride, dutasteride, doxycycline, hydroxychloroquine (Plaquenil), isotretinoin (Accutane), or mycophenolate mofetil
- Steroid injections into the scalp to calm inflammation
- Topical medicines applied at home, such as clobetasol, fluocinonide, tacrolimus (Protopic), pimecrolimus (Elidel), or clobetasol shampoo (Clobex)
- In some cases, hair transplantation may be considered if the disease has remained inactive for a long period
How long will I be on treatment?
Frontal fibrosing alopecia is a chronic condition that requires careful monitoring. Many patients remain on treatment for several years. If the disease appears stable and is no longer spreading, medication doses may be gradually reduced or even stopped. However, if there are signs of new activity, treatment may need to be adjusted, with higher doses or different medications prescribed.
